my grandfathers citation for the D C M

H i have looked everywhere for the citation of my grandfathers medals from ww1, his name was j h barber and at the time he received the d c m he was a private but after was upped to a sergeant,
his army number was 403156 and he was in the r a m c.
if anybody could help i would be very grateful
