Workhouse observations

Can anyone help me read what this says?
Looks like the second word says
head
but I don't know what the first says. The remarks is under the heading:
Observation on condition at time of entry
. Other notes include ailments like bronchitis. He was admitted to the surgical ward, so I assume he had some sort of injury.
Here's the link to the original document:

(it's the first entry).
Any help is much appreciated.
