Help with second line, please?

This is a snip from a military record of a man b. 1842. 

I can see the first line reads "Silver Medal for 'Long Service and Good Conduct' ", but I'm not entirely clear what the second line is all about unless it's just a record of where in the Army's papers the LSGC award was recorded.

Can anyone help, please?

Guess work -
High Grade Government Office dob 13 November 1878

though you say he was born 1842

Sorry but that guess is incorrect! It's Horse Guards General Order. Horse Guards was the alternative and more usual name for the War Office. And yes, it's the Order authorising the award of that particular LS&GC medal. It would have come from the Adjutant General's office.
