I think it's the terminology that may be confusing people. The example in post 7 is a medal index car or MIC (because they were records held in a card index) in the Medal Office. The example in post 9 is a page from a Medal Roll, that is, a nominal roll consisting of a series of pages for each Regiment/Battalion and bound into a larger volume.

Double checked at TNA & they only have what you have so
it looks like he slipped through the net.
It did happen so not unique.
Looking at the Middx. Medal Roll It seems there are a few missing.

Thanks Jim1 that's what I was looking for him.
I thought his 1914/15 Star would have been under a 4 digit number not 6 digit

292116 on this list shows his 3 medals under his new number.
292117 on the next page shows his 3 medals under his old number.
292110 doesn't have an MIC under either number.
A right old mess.

Hold the page.
I don't know if this is a red herring but it looks like a possibility.
I'll explain, your man should have the pre 1917 number from 5149-5165
as the new numbers were sequential.
I have found a George Horne (assuming his name was George)
with the number 5161 who according to his Medal Roll was in the
right place at the right time.
It doesn't show his Middx. Regt. post 1917 number but the London Regt.
may not have been aware of it & he was probably not included on the
Middx. Roll because he had transferred.

Private G Horne 6892 served during the 2nd Boer War with 3rd Bn the Duke of Cambridge's Own (Middlesex) Regt.
