It might be Emphysema ,a serious breathing illness.
There is a skin condition rarely heard of these days Erysipelas ,not sure who filled such forms in ,there are often some spelling  mistakes ,so perhaps medical orderlies,as a Dr would spell correctly.
As it says it next to Distinctive marking I see why your immediate thought was a skin condition .
You often hear people say “ Empysemia “ and “Prostrate gland “,for Prostate gland, so perhaps a simple spelling mistake .
The patient would not usually die from the skin condition but Emphysema can contribute , putting strain on the heart .
Viktoria.

I agree with Sue, Emphysema Scar R, could mean right lung.
Just a guess.

Thank you everyone.
Looking at this further (and just in case anyone searches for this again!) there is such a thing as

Facial Emphysema

Caused by injury or a dental procedure and is caused by air being trapped in the innermost of the skin's three layers.

So guessing my chap had this and his recovery resulted in a remaining scar on his face

The first word might be “ empyema” which is apparently a purulent pleuritis which would need draining and this would leave scars
