      could some kind person please decipher the writing on this form as it would mean a lot to me.

Most of the first part of the Surgeon's Certificate:

Sergt James Morrison

...Roscomn Militia was severely hurted in his

left shoulder & left side of his Thorax by assisting in put=

ting Baggage on the Transport returning from England which

repeatedly suppurated at long intervals [?] has rendered his

left arm [near?] powerless - was also severely hurted in the

Lumbar Vertebrae by part of a House falling on him when re=

turning to his Quarters from duty on the 5th Novr: 1828 which

has rendered his left lower extremity nearly powerless

Perhaps:  ...suppurated at long intervals since & has rendered his...

A bit more:

I certify that Sergt James Morrison was orderly Sergeant

on the 5th November ^last^ and was returning from his duty

when the house fell on him, which has [disenabled?] him, and

killed another person

The surgeon is:  Lodge Hall M.D.
