Worcester County Asylum, Powick Hospital

I have an entry for admission on 28 March 1881, and death of a patient on 22 December 1881, from Ancestry, for one Phebe Bache:

I have also discovered her case notes; they can be accessed from here:

and following the link to search the database (uncomfortable reading, beware). At the end of the notes, her death is recorded, but I am unable to find a corresponding GRO index entry. Were deaths in the asylum recorded on the GRO, and if so, where is the entry? I've checked q4 1881 and q1 1882 without success. I'm also not quite sure what the last line says on the attached image.

Was the Powick Hospital in the Upton registration area?

Powick came under Upton on Severn 1837-1952
