If you Google Queen South Africa Medal you will see dozens of illustrations. The ribbon has a broad central stripe with narrow navy strips either side  and red  stripes a little wider on the outside edges.

 Sometimes  the ribbon can be confusing in old monochrome photos, the light colours show as dark and the dark as light. No one could work out the ribbon on my grandfather until I found his awards in the Royal Engineers Medal Roll.

Medal ribbons were often awarded Head of the Medal. The ribbons are warn every day, whereas the actual Medals are only worn on certain occasions.

I presume you meant 'ahead of the medal'.

Yes, 'for special occasions' would explain the absence of medals in the photo.
