Ellen M Rudge's disappearing act

I am hoping that someone may be able to help find out about Ellen M Rudge.
She was born 1899, Wednesfield, Staffs and married George V Hazelwood in 1917 Tynemouth.
I have found her in the 1901, 1911, 1921 Census and in the 1939 Register.
In the latter, there is a change of surname to McCulloch with the date 24/1/1957 and note Half med? It is faint but legible (RG101/1070H/006/16).
I have looked through the marriages between Hazelwood or Rudge to McCulloch for the period 1941-1958 and drawn a blank. I have searched for a death with Hazelwood or McCulloch but none seemed to fit. Has anyone any suggestions?

On the 1939 register there is no crossing out for the Ellen M Hazelwood I found. It is her daughter Vera Maud born 1922 who has a red line through her name and Ward substituted.
Vera Maud's birth reg on the GRO shows her mmn as Rudge.

Wow thank you for that information. I didn't check the Scottish records. I wonder how she met William McCulloch. There wasn't any oral family history suggesting that.
