Hi everyone, I got a death certificate today but I cannot make it all out. It is for Benjamin Reeves. The first picture has his date of death as 21st December but I cannot make out his address in Oldham. The second picture is for his cause of death and the informants details. I cannot make out the cause of death or the address of Caroline Reaves, the informant.
Can anyone help please?

Cause of death "unknown, not certified, no medical attendant"

As for the address, it might say "Lyon Lane"?

I agree with bbart for the cause of death, and partially for the address. After looking in TNA's archived address list for the 1841 census, I think it's probably Lyon Dam (written Lyondam).

more: FindMyPast address search shows that Lyon Dam/Lyon Dam Street was an address from 1841 through to 1939. The last newspaper hits are in the late 1940s.
