Still have a few gaps, but most of it is there, a summary of what is still missing:

16 February 1905: She is clean and tidy and makes her own bed but is very noisy especially so at night. [Describes?] voices [from within?]. H good.

21 April 1905: Qts. "own thoughts bother me at night  D[octor], because  I [speak?] my thoughts aloud ….annoying    thoughts”. Speaks in a very childish and feeble-minded manner. Memory impaired. Health good.

Think we have everything else.

Here's what I have so far.

1 December 1904: She is [?] and at time noisy and abusive – her memory is impaired, and she gives a poor account of herself. [Has hallucinations of hearing?].
16 February 1905: She is clean and tidy and makes her own bed but [?] especially so at night. [?] [?] voices [?]. H. good.
21 April 1905: [?] “own thoughts bother [me? Her?] [?] at night [?] because [? ?] thoughts [aloud?] [?] thoughts” looks like quotation marks in this one. [?] in a very childish and feeble-minded manner. Memory impaired. Health good.
1 June 1905: Transferred to the annexe on this day.
1 August 1905: Suffering from mania: she is excitable, talkative and noisy, and has hallucinations of hearing. She is in good bodily health and condition.
1 August 1907: Chronic mania. She is garrulous, excitable and at time noisy and abusive. Poor health.
27 October 1907: notice of death. Eleanor D’Ovis, received 31 Aug. 1903, died 27 Oct 1907. [Statement?] appeared cause of death: cancer of uterus. Whether or not ascertained by post mortem examination: no. Time [?] affecting? the death: 9.15 am, none. Duration of disease: several months. Name of person present at death: A. Stockham, attendant. Whether or not [?] [?] was applied to deceased – no.

Can anyone help me read these asylum case notes, please? Any help is much appreciated!  :)

Here's what I have so far:

First image:
18 [referring to 18 Sep 1903] she shows no improvement mentally, is dull and confused [?]

25. She [remains?] dull incoherent and can give [not?] little account of herself. Fair health.

Oct 1. She has [improved?] of late [?] [quite?] [?] dull and demented. Fair health.

Nov 1. She shows no further improvement mentally. She is dull weak minded [incoherent?] and cannot give [?] account of herself. Fair health.

Dec 1. She shows no mental improvement. Is dull weak minded. No memory. Fair health

Jan 1. She remains in the same mental [?]. She is still dull weak minded [?] is at present [?] [?] with gastric pain. Fair health.

Second image:
Feb 1. She remains [??]... herself in the wards. Fair health.

Mar 14. She is noisy and excited and talks incoherently. Her memory is impaired and she cannot give a good account of herself.

Apr 1. She shows no mental improvement. She is [noisy?] and incoherent still. Fair health.

May 1. She shows no mental improvement. She is [? ?] and incoherent. Fair health.

June 8. She is dull and [?] and cannot give any account of herself. Fair health.
