It looks like Mclellan to me, the tail which originally looked like a y is the J from James the surgeons first name.

Further down the page on the 7th of November there are another set of initials M.A. perhaps a commonly used abbreviation for Mary Ann or Margaret Ann.

The regiment is the 53rd YS bn Sherwood Foresters, the YS stands for young soldiers.

I have checked the 1861 census and the following family is living at 133 Manse Street Renfrew,
James Cairns 27 boilermaker head, Agnes Cairns 30 wife, John Young 15 stepson, Hugh Young 11 stepson, Mary Young 9 stepdaughter, Andrew Cairns 5 son, Marion 3 daughter, James 1 month son.

This look like the right family, and I think confirms Cairns as Agnes's surname.
