I have another such occurrence in my tree.
A lady who dies in the 1990s.
Found both under her 1st married and her 2nd married name on the index.
The actual cert said:
Evelyn (2nd married name) formerly known as (1st married name)

 "My dear, I think the English pronounce it 'appiness"  

I'm afraid of no ghost

just to bring this thread up to date, Annie William married James preesdee  drinkwater in 1915 Worcester, hence the preesdee drinkwater williams names on the John Nolan record.
