This has opend my eyes, a week ago I didnt even know these records existed. I will have to look up some of these medical terms!

If I were just squinting at that word, I'd think "fine" but given that the first letter can be seen in other places as a lower case P it might be something like "pure" although I'm not entirely sure why someone would call heart sounds "pure"...

A little tweak:

"Pulmonary percussion on the left side is
quite normal & the [R.M.?] though somewhat
puerile is quite free from rales."

And I think Ayashi is right about this part:

"Cardiac dulness is normal and the cardiac
sounds are quite pure...."

I had thought it was "good", but the writer has a particular way of forming his p's - and "pure" was (is?) a common descriptive term for cardiac sounds - at least the old medical texts on Google Books indicate this.

I thought it might have said "normal" but you thought it ended in -ed and I squinted and agreed, lol, I need glasses so I thought you'd know best!  ;D
