Mothers Surname on Birth Certificate- completed

I obtained a pdf of the birth certificate of my grandmother from Irish World Family History Services.
On it the mothers surname is different that of the father and child.

Does this mean it is :

(1) Her Maiden Name.
(2) Unmarried
(3) No Idea...

Civil birth certificate asks for mothers name. If married that column will read like this:
Jane Johnston
formerly
Smyth
meaning that her maiden name was Smyth and present name Johnston. However, if you've gotten the certificate from another source I'm not sure what way the information has been transcribed.
Away sorting out DNA matches... I may be gone for some time many years!

I wasn't sure as i'm pretty sure that they were married as there is a record 4 years earlier.
maybe this record isn't the right one?
I agree on all my english research it will always read xxx formerly yyy.
I wasn't sure about the irish way.

Your 1st post mentioned getting a pdf of birth certificate- is it a scanned image of the original or a form that Irish World uses?
