Well I don't speak a word of Polish, but I can help a bit.

I think this is written in Ukrainian; Cyrillic Alphabet.  Polish uses the Latin alphabet as far as I know.

Thanks for that, we thought that as the mother came from Poland that it would be Polish.

I see dates in the document around 1902-1909;  at this time, territory that is now Ukraine may well have been in Poland - the borders have been very fluid over the last hundred years.   It's written in Ukrainian.

I've found a way to do it but it's quite time consuming!  If you Google 'language tools' and select Ukrainian to English, then select the little keyboard to get a Cyrillic keyboard up, it's possible to type in the letters and get a translation.  So far I've done:

Архівна Довідка
В документах архіву є відомосту

which translates to

Archive Help
In the archives there is information

I have to go out shortly but will do some more later if someone else doesn't do it in the meantime  ;)

Don't you just love t'internet?  ;D

Archive help
In the documents archive there is information about Lukashuk Artemiy Grigorovich, born 1875, with his wife Anastasia Antonivna, born 1870, and their children, natives of the village of Hopniv in the former district of Lutsk, Volhyn governorate/province: son Pavl, born 23.12.1902; daughter Anna, born 17.11.1904; twin daughters Eva and Oleksandra/Alexandra, born 03.02.1909; son Antoniy, born 02.08.1912, and daughter Nadia, born 29.01.1915.
No information was found about Lukashuk Adam Artemovich, born 1910, about his service in the Air Force or about other members of his family.
We take this opportunity to advise you that a substantial amount of documents was lost or removed from the Ukraine in 1944 during World War II. According to the present administrative territorial divisions of the Ukraine, the village of Hopniv is currently part of the municipality of Trostyanets in the raion (department) of Kivertsi in Volhyn Oblast (province).
