Seeking birth certificate for John Christopher Keogh Cullen

Hello everyone

We are trying to obtain the birth certificate for John Christopher Keogh Cullen born 23 December 1922.  We have done the usual Ancestry etc searches and have found the index details below

Date of Registration: Jan-Feb-Mar 1923
Registration District: Dublin North
Birth Country: Ireland
Volume: 2
Page: 374
FHL Film Number: 101229

There is no image available. And when we attempt to order the birth certificate online we are required to complete the father's and mother's birth names, of which we do not know.

Please could anyone offer any suggestions or help as to how to progress - apart from us getting on a plane to Dublin! :-)

Do you need an official cert or will a photo copy do ?
Research cert only cost €4 but must be ordered by fax or post the cert will be emailed if you wish.
An official cert costs more but will have the same info
How to order both is on that link.
Without needing the parents names.

Thank you Sinann, we have already accessed these links but are struggling to complete the application without the mother's birth name - any ideas of what to do without this information? If we type "unknown" in the compulsory field will that be futile? Thank you :-)

For a photocopy you need to click on the word 'here' on the page, it brings you to this form

Where does the Keogh come from?
Usually the mothers maiden name in most cases.
