A bit of a tidy up needed

I've had a go at tidying this but just seem to make it worse...sorry not very good with photo imaging stuff. Can anyone help please

Cleaned up the large spots with the spot cleaning tool PSE3, then applied Neat Image filter and a sepia tone.
Not sure it's much of an improvement though...I'm sure the pros on this board can do much better!

A black & White version, cleaned, clarify filter, soft focus filter, slight crop using PSP9.
