Dreadnought Seaman's hospital Admission and Discharges

I have an entry from this set of records for an ancestor of mine and I can't decipher what these two bits of information say.
The first photo is which ship of employment and the second is nature of complaint.
Can anyone help?!

Or 'complaint' as in complaining about someone or something?
