an old Benevelonent Society death information

hello everyone i have an old document detailing the death of a relative but alas i can't understand it at all if some kind person could read it and provide me with the information i would greatly appreciate it thanks

Apologies for asking what may, to you, seem a silly question.  But are you asking for a translation of the document? (i.e. you cannot read what it says)

Born N S Wales.   R.C.   Horse Dealer  ___  Single
Has relatives but does not know where they are or their addresses
not having heard from them for over 30 years  -  no money
or property. Has been an abstainer all his life -
Previous admission 15th September 1885 and was discharged
after 2 years residence -
Certified suffering from "impaired vision" from a fall he had
from riding a Race horse, injuring his head in the front
part in the year 1886 -
About 2 years ago had another injury to his head from a
limb of a tree -
Went to Machamp(?) Hospital after the last accident having
___ injured his foot, and it mortified, causing the toes
of his right foot to be amputated.
- - Has been getting advice from Dr Taylor Brisbane(?) for his eyes.
who said "It was a matter of time"
Has ___ in ___ and defective vision
M ward 20   Comfortable

From  a news item dated 1914, we see that  James was many years in the Dunwich  Benevolent Asylum  where he died during the month of February
