I beg to differ, I have now seen 2 Victorian Marriages that contain that information, both dated 1854.
One of the marriages has the children columns filled in for the groom.

I'm hoping if there is a blank image of a Marriage registration or Certificate that also includes columns for children born and deceased for either Bride or Groom.

No, there would be no such form or certificate.
It is not relevant required information at marriage
A death certificate may show children born of the marriage which the deceased person was in at death. Not of previous relationships.
