Thanks for replying Neale,
I'm hoping if there is a blank image of a Marriage registration or Certificate that also includes columns for children born and deceased for either Bride or Groom.

.. a Marriage registration or Certificate that also includes columns for children born and deceased for either Bride or Groom.

You would normally find that information on a death certificate.

Here is an example of a marriage record from the 1880s that provides an opportunity to list children.

Civil registration in Victoria started in 1853, prior to that there are only church records.

Mick

No, there would be no such form or certificate.
It is not relevant required information at marriage

A death certificate may show children born of the marriage  which the deceased person was in at death.  Not of previous relationships.

I beg to differ, I have now seen 2 Victorian Marriages that contain that information, both dated 1854.
One of the marriages has the children columns filled in for the groom.
