That's not a Medal (Index) Card.
It's a page from the Regimental Roll.

Which column are you referring to?
Garrad (Suffolk, Essex, Somerset), Crocker (Somerset), Vanstone (Devon, Jersey), Sims (Wiltshire), Bridger (Kent)

In blue states that the medals were returned & re-issued 9/1/24.
He appears on the Cheshire Regt. Roll as 86316.
The Black sea reference is a bit of a mystery.
It's possible he re-enlisted post war & this is where he was when
the medals were returned but that's just a guess.

I think he may have rejoined and been sent by Lloyd George as a Peacekeeping force 1923/24 to oversee the Greek withdrawal and setting up Turkey in 1924
