Asylum admission notes

Hi,

I was wondering if anyone could help me read some of the notes for an asylum admission, please? Any help is much appreicated!  :) Here is what I have so far.

First image:
Liverpool: of a [?] tendency: for the past 15 years, patient has been drinking heavily: [although?] [?] family [?] health : No previous attacks : no history of [?] mental anxiety of shock : [?] fair [?].

Second image:
Father: Liverpool, builder [?]
Mother: "                         "

Third image:
Teeth: [defective?]   Tongue: 7 [?] and [tremulous?]
Genito-Urinary system: 1020 acid, no alb. no sugar
Muscular system: fair.     General condition - Fair.


Fourth Image:
She was admitted in a dull and confused state. She was talkative and restless and paid little attention when assessed. Her power of intelligent conversation was impaired and at times she became [?]. Her memory was seriously impaired and she [?] not five a reasonable account of herself. Apparently she has no delusions or hallucinations.
She slept for about 3 hours during the night and became restless afterwards. She is clean in her habits.

I don't know what this part is:
no history of [?]     

2.
father is a builder, quiet temperment, and checked off as sober.

3. teeth defective, tongue "furred"  [the F matches the F in Fair on the same image.

4. at times she became irrational ...
and she could not give a reasonable account of herself

I think it says "always enjoyed fairly good health"

I think this word could be anything but I'm not certain at all.

"ongoing" makes sense.
