I have some more notes I need help with, can anyone assist?

Here's what I have so far.

1 December 1904: She is [?] and at time noisy and abusive – her memory is impaired, and she gives a poor account of herself. [Has hallucinations of hearing?].
16 February 1905: She is clean and tidy and makes her own bed but [?] especially so at night. [?] [?] voices [?]. H. good.
21 April 1905: [?] “own thoughts bother [me? Her?] [?] at night [?] because [? ?] thoughts [aloud?] [?] thoughts” looks like quotation marks in this one. [?] in a very childish and feeble-minded manner. Memory impaired. Health good.
1 June 1905: Transferred to the annexe on this day.
1 August 1905: Suffering from mania: she is excitable, talkative and noisy, and has hallucinations of hearing. She is in good bodily health and condition.
1 August 1907: Chronic mania. She is garrulous, excitable and at time noisy and abusive. Poor health.
27 October 1907: notice of death. Eleanor D’Ovis, received 31 Aug. 1903, died 27 Oct 1907. [Statement?] appeared cause of death: cancer of uterus. Whether or not ascertained by post mortem examination: no. Time [?] affecting? the death: 9.15 am, none. Duration of disease: several months. Name of person present at death: A. Stockham, attendant. Whether or not [?] [?] was applied to deceased – no.

Agree with “hallucinations of hearing
16 Feb. But is very noisy.
April 21 maybe that small word is ‘Qts”.  ie Quotes followed by her own words
   “own thoughts bother me at night  D[octor?], because  I [? ] my thoughts aloud ….annoying    thoughts”. Speaks in etc. as you have
1 Jun. there is no “on” in that line
1 Aug.  as you have but - is at times noisy
27 October   Apparent cause of death (not appeared)
     Time & any unusual circumstances attending the death
     Whether or not mechanical restraint was applied

Just that one word on April 21st I am not sure of. I thought it could be speak, but it isn't much like what is clearly  speaks on the next line.

Still have a few gaps, but most of it is there, a summary of what is still missing:

21 April 1905: Qts. "own thoughts bother me at night  D[octor], because  I [speak?] my thoughts aloud ….annoying    thoughts”. Speaks in a very childish and feeble-minded manner. Memory impaired. Health good.

Think we have everything else.

Describes makes more sense, but it looks more like Denies to me.

I think that might be 'Pts'; an abbreviation for 'Patient is' or the contracted version 'Patient's'

16 Feb: I think it's "Denies voices for months", as mckha suggested. (The 1 Dec entry mentions hallucinations of hearing, but here, two and a half months later, she denies that it's been happening, or if it has, that it's been going on for so long.)

21 Apr: "I express my thoughts aloud"
