I'm having a spot of bother with a couple of words, so we'll have to wait for davecapps to come along.  ;D

Here's the rough transcription:

I am having trouble with the lady's second name, the 2 sections with
der Persönlichkeit nach xxxxxxxxxxxxxxx
-------------- unerkannt,

and the name of the road in which the lady lived.
